About: The Community Family is dedicated to providing high-quality adult day health services designed to keep adults in their homes and communities as long as possible. Our services are based on a deep respect for the dignity, self-esteem and vitality of all those we serve. We are committed to improving the quality of life for both our participants and their families.
TCF’s Philosophy of Care is rooted in a deep respect for the dignity and self-esteem of each individual. Our programs emphasize feelings of accomplishment, self-worth and belonging. Individualized care plans recognize the unique circumstance of each participant and their family.
Services: The Community Family provides a holistic day program comprised of nursing services, therapeutic activities, social work services, nutrition, direct care services, socialization and transportation. The Community Family is a mission-driven nonprofit organization that has provided adult day health services for more than 35 years. We provide care to frail elders, adults with disabilities and those with Alzheimer’s living in Lowell and all surrounding communities.
Our Lowell adult day health center specializes in caring for those with memory loss, most often Alzheimer’s; however, the center also has participants with other chronic medical conditions. Participants attend the center on a scheduled basis and benefit from the care and supervision that we provide. Participants enjoy a home-like environment which most refer to as “the Club”.
Every day is a happy, positive experience where participants may engage in musical activities, discussions, crafts, physical and cognitively stimulating games and special celebrations.
Our mission extends beyond the doors of our center. We hold a monthly Caregiver Support Group and are available to provide training and education on both elder care and memory related issues.
